Individuals who are homebound due to illness, injury, or disability and require skilled nursing care or therapy services prescribed by a physician may qualify for services from our CHHA.
Certified services are typically covered by Medicare, Medicaid, private insurance, or out-of-pocket payments, depending on your eligibility and coverage.
The frequency of visits from our nurses or therapists depends on your care needs and the treatment plan prescribed by your physician. Visits may range from daily to weekly or less frequently, as determined by the care team.
While you may express preferences for caregivers, the availability of specific caregivers may vary based on scheduling and staffing. We strive our best to match you with compatible caregivers whenever possible.
To start services, you typically need a referral from your physician, who will prescribe the necessary skilled nursing or therapy services. We will then conduct an assessment to develop a personalized care plan for you.
We prioritize the safety and security of our clients by conducting thorough assessments of the home environment, providing education on fall prevention and home safety measures, implementing protocols for emergency situations, and more.
