The GUIDE Program is a new Medicare-covered service that offers personalized support to people living with dementia and their caregivers. It includes help from a dedicated care team, 24/7 access to support, and tools to make daily life easier—so you don’t have to face dementia care alone.
Yes, the GUIDE Program is fully funded by Medicare and comes at no additional cost to you. Your Medicare benefits will not change, and you can still see any doctor, specialist, or hospital that accepts Medicare.
You’ll get a custom care plan, regular check-ins from our care team, and access to dementia-trained specialists 24/7. If you’re a caregiver, you’ll receive guidance, training, and emotional support—plus help when you need a break through our respite care benefit (up to $2,500 per year).
To qualify, the person living with dementia must have Original Medicare (Parts A & B), show symptoms of dementia, and not be enrolled in hospice, the PACE program, or living in a nursing home. Our team can help determine eligibility and get you started.
