Frequently Asked Questions for Referral Partners

  • The GUIDE (Guiding an Improved Dementia Experience) Program is a federally funded Medicare initiative that provides dementia-specific care management, 24/7 clinical access, caregiver education, and community-based support. It helps stabilize patients at home, reduce unnecessary hospital or SNF admissions, and improve long-term outcomes.

  • Emerest is one of a select group of providers approved to deliver the GUIDE Program in New York, with the potential to expand nationwide. With 30+ years of home care experience, Emerest offers personalized care coordination, caregiver support, and innovative tools like Emerest Connect Live TV to enhance dementia care delivery for both patients and partners.

  • Referring to Emerest’s GUIDE Program can help reduce readmissions (30/60/90 days), minimize service escalation requests (like increased home care hours), and ensure post-discharge continuity through real-time alerts, 24/7 support, and seamless coordination. For MLTCs and hospitals, this directly supports value-based care metrics.

  • No. The GUIDE Program is 100% covered by Medicare. There is no cost to your organization or the referred patient, as long as they meet eligibility criteria (Traditional Medicare Parts A & B, dementia symptoms, and not enrolled in hospice, PACE, or residing in a nursing home).

  • Our care navigators and clinical staff work closely with your care teams to share relevant updates, ensure proper follow-up, and maintain open communication. We also offer hospital and SNF admission notifications, help transition patients to long-term care if needed, and keep you informed every step of the way.
